If you are injured because of work-related activity, it is very important that you follow the steps below for injuries that are not life threatening. In emergency medical situations that are concerned with the preservation of life and limb, seek immediate medical attention such as by calling, or having someone else call 911 or other appropriate means.
The employee should immediately notify his/her hiring authority, supervisor, or the North Carolina Administrative Office of the Courts (NCAOC) Safety & COOP team. This is the first step in initiating the Workers' Compensation process.
- If the injury requires minor medical treatment, you can find an authorized local network provider and/or pharmacy, by using the searchable database on the third-party administrator's (TPA) website. Instructions can be found here.
- Choose from the selection of best matching parameters
- Choose urgent care unless the employee has a life-threatening issue
- Print a copy of the Initial Medical Treatment Guide and the First Fill Prescription Card to present to the medical facility selected. A copy of the completed Initial Medical Treatment Guide should be submitted to Judicial.Safety & COOP, along with any other medical documentation provided to the injured employee.
- In the event an injured employee seeks medical treatment without the knowledge of the hiring authority or supervisor, notification of the treatment should be reported within one scheduled workday following the injury. This should only occur when an injured employee does not have immediate accessibility to their hiring authority or supervisor.
- An injured employee should not use their private health insurance when seeking treatment for injuries sustained in the workplace.
- After necessary treatment has been received, the injured employee should meet with the hiring authority or supervisor to complete incident report forms. An incident report form should be completed by the injured employee, hiring authority or supervisor, as well as any eyewitnesses to the incident.
- The hiring authority or supervisor will submit all incident report forms and medical documentation/ discharge paperwork to the (NCAOC) Safety & COOP team for evaluation and processing within 24 hours of completion at:

- After initial treatment, all subsequent treatment and Workers' Compensation claims must be approved by the TPA as the third-party administrator. Employees who have any questions regarding compensability, treatment, or billing should contact the TPA assigned to handle the claim as identified on claim correspondence.
- All injuries must be reported within 30 days of occurrence to be compensable. Reporting an injury does not mean automatic coverage under the Workers' Compensation program. The TPA will notify you if your claim is denied.
- It is our intention to ensure that you receive medical treatment as soon as possible. The Safety & COOP team is designated to assist you with any questions or concerns that you may have regarding on-the-job injuries. Should you need assistance, call the Safety and COOP Team at 919-890-1100.
- If an employee experiences a work-related incident resulting in bodily injury and declines the option of applying for Workers' Compensation benefits, the following forms may be utilized:
- The Waiver of Medical Treatment form is used when an injured employee declines to be medically evaluated for a work-related injury, although it has been requested.
- The Waiver of Workers' Compensation Benefits is used when an injured employee acknowledges a work-related incident occurred that resulted in bodily injury and waives all rights to any possible Workers' Compensation benefits.
