Employee Certification of Aggregate Service
		This form must be completed by all permanent employees. If you have prior State and/or local service you will want that service credited to your total years of service. This form enables the Human Resources division to verify your past service and ensure that it is properly credited to your records. The information on this form also provides credit toward your vacation leave, longevity, and service awards if you are in a leave earning position.
